1. How do they start?

The osteoarthritis is in general developing slowly, months or even years before the first painful symptoms appear and lead to gradual painful limitations of the joints. The most frequent joint disorder among the polyarthritis illnesses is the rheumatoid arthritis ( RA), which occurs often fast within some weeks, in other cases it develops in a latent way, slowly, in months, causing articular pain, pressure sensitivity and swells up. Lack of professional medical aid leads to irreversible damage of the joints. The symptoms and the limitation of movements occurs in the case of discus hernia very rapidly, usually in couple of seconds.

2. How to treat?

An effective treatment of the rheumatic diseases that is the increase of life qualities can only be achieved through a close cooperation of the patient and the doctor. Now days with the rapid increase of scientific discoveries (such as biological therapies) an immense change can be pursued in the life quality of patients suffering from autoimmunopathy. 3. Risk factors

In the case of osteoarthritis the primary risk factor is the familiar history. Osteoarthritis is most frequently developing with aging and effects middle age population whose family often diagnosed with this disorder or people whose work or life style burdens more their joints than average, perhaps those who suffered serious injuries. Often osteoarthritis caused by a development disorder of joints, but diabetes or alcohol addiction causes the articular disorder. Skin illnesses (eg. Psoriasis) might also result arthritis. Certain illnesses are more frequent with men, while others with women. Women tend to suffer more often from the osteoarthritis of hands, while men from ankylosing spondylitis or disc hernia. Osteoarthritis influenced by family tendencies, gender related, more often of women patients, a risk factor is the thin body structure, lack of physical exercising, light skin colour, ovariectomy, consuming alcohol, smoking, taking medications on permanent base etc.

4. Genetic background?

In most cases of this illness familiar dominance can be observed, but genetic determination has not been so far scientifically proved. The difference between them is that in case of the familiar dominance the influence of the environment and life style are also an important factors in causing the particular illness. By studying family-trees dominance was discovered with e.g. inter vertebral disk hernia, RA, or osteoarthritis, but only with AP the genetic deterioration factor was proved undoubtedly.

5. Damages in Osteoarthritis?

In case of osteoarthritis the cartilage between the bones becomes thinner, and in most cases it cannot be told why. The dysfunction of nutrition of the cartilage can accelerate the process. The cartilage becomes thinner and thinner and degranulated, while pathological changes in subchondral bone can be detected. In most cases it cannot be explained while the right hip joint is effected and not the left. It can also happen that the very first pathological sign won't occur in the damaged joint itself but in areas above or under it. The hip osteoarthritis might start with pain occurring around the waist or the knee. If the nutrition of the cartilage cells worsen, the process accelerates. The effected joint might suffer inflammatory reaction and the joint will be filled with fluid which worsen its condition.
 
6. Low back pain?

The space between the vertebrae filled with a flexible material, the inter vertebral disk. It does not just fills up the space between the neighboring vertebrae, but also because of its flexibility swallows hits (eg. jumping from somewhere) that is protects the vertebrae. However, if the micro architecture of the disk changes (eg. loses water), disorder will occur, in worse case the disk becomes herniated. In these cases the disk loses its flexibility, protrudes, becomes herniated and the nerve-roots of the spine might get compressed causing irradiated pain (called: radiculitis ) down even to the ankles.
Low back pain occurs in case of simple osteoarthritis in the small vertebral joints as well. There is no hypaethesy and reflex abnormality in this case. The effected spine area becomes painfully limited of movements, the muscles along the spine will undergo muscle spasm and the pain radiates to the shoulders or the hips and the patient needs a cane to walk.

7. Location of Vertebral Pain?

The pain appears most frequently in spondylopathy ( small joint osteoarthritis in the spine) and discopathy of the neck and the lumbar part of vertebral column. From 7 neck vertebrae the lower 4-5 and from the 5 lumbar vertebrae the lower 2-3 will become problematic the most of the time, and can be detected rather precisely at examining the patient. Additional examinations are also carried out with X ray measurement describe the bone abnormalities and MRI shows the soft tissue – lice vertebral disk – discrepancies. These examinations will reveal more useful information such as the exact location and size of the herniated disk. Further more, in case of the possibility of bone cancer or bone metastasis a scintigraphy might be very informative.
